Dear Veteran:

I am your newly appointed National Service Officer, serving all veterans and their family members in Missouri - mainly in the Southwest Missouri region.

You do not have to have a Purple Heart for me to assist you – I am here to help all veterans. If you want to see me about making a claim to the VA for benefits, some of the information you need to bring along is as follows:

-         Separation papers:

   - DD Form 214 or DD215 - for all periods of service,

         - or for WW II vets, a WD Form.

If, for some reason, you cannot find your separation papers, I can also assist you in getting these.

If you are married and have any children under the age of 18 living at home, I will also need your:

- Date of marriage

- Social Security numbers, and dates of birth - for all

- Names and addresses of private doctor’s, if not receiving care with the VA.
